A 22-years old female elite runner with persistent anterolateral pain on the left ankle.

Picture 1 + 2: On the MRI we see intra-articular fluid(as a sign of irritation) with an intra-articular fat pad, just proximal of the ATFL, connected to the talus with a small connecting septum.

Picture 3: During arthroscopy the connecting septum(marked in red) and fat pad were detected and removed.

During the post-operative checkup the pain was no longer present.
